Long Term Conditions Telephone Support
This is a free service that supports people with Long Term Heath Conditions or those people who may be at risk of loneliness and social isolation. There is no charge or committment and the support can be for up to three months.
All the staff and volunteers at Health Captains all have lived experience of health conditions themselves or have supported family/friends and other people with them. The service offers practical and emotional support to help you through your care journey and beyond.
